Another recommendation, but I havent examined it yet is:
A Practical Guide to Culture, by John Stonestreet, (he has worked heavily with Summit Ministries and is President of the Colson Center for Christian Worldview) and Brett Kunkle (of Maven Ministries). Both are phenomenal writers and have worked with teens and parents and sharing worldview with them. The book is written to a parent/grandparent audience and there is a student edition written for younger kids.
It is said to build the foundations of a worldview in addition to addressing specific topics and gives lots of helpful application points to directly implement into a believers life.

*It would be a good Idea to utilize all of these resources for a well rounded course for your church congregation, teaching age appropriate on the topics, but also for parents to teach at home too.
